Symptom narrative

9/8/2025 3:15 Vaccine given. Shoulder and arm started to hurt about 3 hours after vaccine was given (6:15pm), by 3am I was in so much pain I needed to go to the Urgent Care. Was given 600mg of Ibuprofen and Hydrocodone for pain. Arm proceeded to hurt and unable to move it without severe pain at times. Another follow up on 9/15. By 9/17 feeling a little better but not great range of motion. Pain comes and goes. By 9/20, the pain was so bad I needed to take the hydrocodone. As of today, 9/22 it still aches, I can not work out, lift anything heavy and certain movements make the pain much worse. At times I will feel a surge of pain down my arm. I can feel one particular area where the injection was that hurts to put pressure on. ** Injection area was so high it was basically at my shoulder. It appears she did not even get it in the muscle.

The rotator cuff muscle bellies are normal. No displaced labral tear. No paralabral cyst. The long head biceps tendon is normal in size and signal intensity and is properly positioned in the bicipital groove. No focal glenohumeral articular cartilage defect. The acromion is type II. Mild anterior downsloping. Mild acromioclavicular capsular hypertrophy and marginal osteophyte formation. Enthesopathic cyst in the posterior greater tuberosity. Elsewhere, the bone marrow signal is normal. There is small volume fluid in the subacromial subdeltoid bursa. No glenohumeral joint effusion. Breast prosthesis noted edge of the field-of-view. Impression 1. Shallow partial-thickness articular surface tear supraspinatus. 2. Mild acromioclavicular osteoarthritis. 3. Mild subacromial subdeltoid bursitis.
