Pt passed out on her chair after receiving her vaccine, while positiong the patient on a flat surfaced while other staff calls EMS, pt had a grand mal seizure upon positioning her on a flat surface, head was protected, safety and privacy provided at this time. Vitals signs in HD are as follows: BP-128/81, HR-105, O2 Sat on RA 99%. When EMS arrives pt is already conscious and coherent, was able to provide her Name, DOB and provided her mother's number. Pt states refusal to go to ER, howerver EMS stated she has to be in the EMS car for assessment and evaluation while waiting for her parents. Patient and parent declined to go to ER and opted to go home. This RN called number provided on file to follow up, mom states pt is terrified of needles, just like the other siblings, mom also states is that another reason why she passed out is because of her constant practice for cheerleading. Pt mom was instructed that if patient had another episode of seizure or if any throat swelling and DOB, she has to go to ED, pt mom voices understanding.
