Symptom narrative

Patient was supposed to recieve an MMR vaccine but inadv3rtently recieved a Menigococcal Conjugate (Menveo), but is was diluted with a sterile dilutent of sterile water prior to administering the 0.5 cc of the vaccine to the R arm as a sub-q injection.
