Administering Proquad (MMRV) vaccine, when I started pushing the vaccine to administered it immediately started to spray out the side of the cap and cap & need came unattached from the rest of the syringe. Patient was not harmed. Due to all the vaccine spraying out, the vaccine was repeated. Sterile dilutant syringe used to dilute vaccine and draw back up into to administer lot # was Y015120, this was where the cap came off. Needle lot number for 25 gauge need was 25D088
