Symptom narrative

On Sept. 22, 2025 I was not away that the Hep A vaccine had expired over the weekend before I had administered the vaccine to the patient. Immediately I notify my supervisor Dr (withheld), as well as the patient parent and explain to the parent about the expired vaccine that was administered and also check on the patient and also to the let parent know to bring patient back to the clinic within 30 days to get another Hep A vaccine.
