Symptom narrative

Within a minute after giving vaccines child broke out in a circular splotchy rash around both injection sites on both thighs and on forehead. No vomiting, no wheezing, no breathing difficulty. Observed in office. Rash went away on its on within minutes. Referral made to Allergy for evaluation prior to additional immunization.

Current illness

had fever up to 103.3 about 2 weeks prior to vaccination. had been evaluated in ER.
