Wrong vaccine given: Patient came with mom and another adult woman on 9/17/2025.Original shot record was provided from another country to the front office staff by the mother. There was some confusion with the record and shots were entered in the database incorrectly. What the school nurse said was needed did not match with what was put in the database, so the HD nurse gave tdap, hep b, and polio based on the historical shot record in the database. Error was discovered, historical record in database was fixed and the patient was back to HD by school nurse get mmrv which is what needed.
