Symptom narrative

approximately 10 minutes after the administration of Varicella and HPV vaccines the client stated she was dizzy and felt nauseous. The clients' lips were pale. The client went to the bathroom and vomited and had a bowel movement.

Clients vital signs were taken. BP - 88/53 HR - 63 and Temp was 97.2F. 20 minutes later the clients BP was 96/65 and HR - 70.
