Symptom narrative

Administered vaccine in morning. Reports no pain at time of vaccination or throughout the work day. Returned home and developed severe numbness in the ulnar distribution of the L hand in the pm associated with swelling. No erythema, no other organ system changes. Presented to occupational health clinic in morning, got ring off the hand to prevent further injury. Will monitor swelling during day with an afternoon recheck. Employee called personal provider and recommended to go home.
