0825 Pt c/o rt outer FA, pen size raised hive, near elbow-red, itchy, and raised, denies dysphagia, BP 126/72, oxygen saturation 98%, HR82, denies SOB, CP. 0827 Pt c/o nausea, dry heaving. Benadryl, 25mg, po, given w/sip of water. 0830 BP 110/70, O2 Saturation 99%, HR 88. 0840 I spoke w supervisor informed of symptoms, recommended not to return to work. 0855 Employee stable, hive flat to touch, pink in color, itching decreased. 0909 Discharge instructions given w VIS statement. Informed to notify doctor. Pt verbalized understanding. Discharged to husband.
