Symptom narrative

The child received MMR, PCV20 and PedvaxHIB vaccines on 09/12/25 at 14:42. Monitored the child for 15 min. after receiving the vaccines with no complaints or complications. Following morning on 09/13/25 Mom noticed the hives on arms, legs and groin area. Mom took the child to UC then ER. The child was given steroid, ibuprofen and antihistamine. The child had fever during ER visit. The child had f/u ER visit with PCM on 09/15/25. Mom stated that the child had also diarrhea over the weekend. PCM has suspicion for anaphylaxis. The child has referral placed to follow up with allergies specialist.
