Symptom narrative

Patient fainted and fell off of his chair immediately after receiving the 4 vaccinations. H regained consciousness after a few seconds but felt dizzy, shaky and was sweating. He was observed for 20 minutes and he recovered. Patient reports fear of needles. His wife came to collect him.
