Symptom narrative

Patient received vaccine, walked out of shot room, reported dizziness, sat down, then passed out in chair. Parents were on the scene to monitor. Gave patient water, patient was good to go after 15 minutes. Parents reported history of syncope with shots and refused ambulance assistance.
