Symptom narrative

patient received immunizations for Flu-HD and Comirnanty Covid on 9/25/25. She states that she woke up during night time with 100.6 F fever and loose stools. she states that she has had similar side effects in the past, She has been taking Tylenol to help with fever and I instructed her to take Advil in addition to drinking extra fluids to prevent dehydration. Her fever has gone down as of this morning and improving.
