Symptom narrative

pt presented to clinic 9/26/25 with local redness and pain to left arm that started on 9/18/25 the day of injection. in clinic today 9/26/25 pt stated redness, pain has decrease and new onset of itchiness. we advised pt to take tylenol for pain, apply Calmoseptine ointment and apply cold compress to area in 15-20 min intervals.

Medical history

Alzheimer's disease. Giant cell arteritis, stage 3b kidney chronic kidney disease. hypertension and postmenopausal osteoporosis.
