Symptom narrative

Patient did not received an age appropriate COVID vaccine dose. Pt received Pfizer Comirnaty 10mcg dose for 5-11 year old. Notified the patient's primary care physician and physician called guardian and discussed the event. Guardian reports no symptoms. Pt has since attended school and done fine.
