Pt came in to ask for a flu shot, it was verbally verified twice with pt before giving him the vaccine. about 30 minutes later wife said that he already had a flu shot and instead needed the covid shot. So patient got 2 flu shots within 3 weeks (verified in system 1st dose was 9/9/25. Went over any adverse events patient may experience and also informed his dr. Pt also got covid shot on 9/26/25. Patient was not upset and understood the side effects he may experience.
