Symptom narrative

Pt came in on 9/27/25 complaining about "numbness around the injection site, difficulty moving his arm behind him or moving it in front of him. He had no complaints about moving his arm above his head. Stated that the area was red with a white dot in the middle of it before he came to the pharmacy, upon my observation I was unable to visually see any redness or the area where the vaccine was given. I have referred him to his primary to have them obtain imaging so we can rule out any possible incorrect administration.
