Symptom narrative

ADE: Injection site reaction x 2 days to left arm with 10cm x 7.5cm erythematous blister. Blister itself is fluid filled. Denies any previous history with ADE to previous vaccines. States that she has had COVID-19 and Tdap vaccines in the past without issues. Pt complains of pain and difficulty sleeping on arm Treatment: Prednisone prescription. Outcome: Unknown.
