On an unknown date, the patient received Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: I used two of the diluents for Shingrix but not the antigen) and inappropriate dose of vaccine administered (Verbatim: I used two of the diluents for Shingrix but not the antigen).
