Symptom narrative

myalgias, headache and nausea starting 24 hours after vaccine. Severe chest pain starting 72 hours after vaccine, abnormal troponin and EKG, focal wall motion abnormality on echocardiogram, normal coronary arteries by cath. High suspicion of focal myocarditis.

20 lb unintentional weight loss starting ~ 3 months prior

HTN, epilepsy, GERD, asthma, depression/anxiety
