Lab data

CT Brain: Loss of gray-white differentiation in the left frontal and insular lobes, likely sequela of an acute/subacute infarction. . No acute intracranial hemorrhage, mass effect, or midline shift. CT Angio Head and neck: Patent cervical and proximal intracranial vasculature. MRI Brain: 1. Diffusion restriction within the left frontal cortex, frontal operculum, and insular cortex, consistent with acute or early subacute ischemia. 2. Punctate focus of associated susceptibility artifact likely representing petechial hemorrhagic conversion. 3. Similar mild diffuse parenchymal volume loss and chronic small vessel ischemic changes. Echocardiogram: 1. Normal LV size, normal wall thickness, estimated EF of 60 - 65%. 2. The mitral valve is normal, mild-moderate regurgitation. 3. Tricuspid valve is normal, moderate regurgitation. 4. No shunt detected by color Doppler. No evidence of A-fib on telemetry, no prior history of a-fib. Video consultation with Tele-stroke neurology.
