Symptom narrative

5th DTaP vaccine was given too soon. The patient had his 4th DTaP on 5/29/25 and was on a catch-up schedule. This nurse did not catch the date and assumed child was in for routine 4 year old vaccines, so Kinrix and MMRV were administered.
