Administration Error. Parent received a message from the medical group orienting to schedule the vaccine, although the vaccine is recommended only for children 6 months and older. Parent scheduled, took the 2 month old patient to the clinic and she got vaccinated despite multiple checkpoints of her date of birth. Clinic also documented the vaccine as given on the left deltoid, although it was given on the left thigh. Fortunately no adverse events were noticed in the 72h following the vaccine.
