Symptom narrative

client complained of right site injection and a Headache. Client was observed for 20 minutes. She left the area and returned to her work area. She returned and stated she had taken aleve without any resolution in her headache. She was escorted to the ER for evaluation. Release from ER with diphenhydramine and hydroxyzine. To follow up with PCM
