Symptom narrative

Menquadfi and HPV 9 were both administered to RD. Both vaccines were #2 dose in the series. Client sat for a small amount of time prior to standing and stated he felt fine. Then walked to the front clerical desk with mom and brother, then collapsed hitting ledge of counter with lower part of face then head made contact with wall.

Lab data

Vital signs were obtain during episode . BP was 145/69 RA, heart rate of 73 and O2 saturation of 97%. Client was talking during BP check. Next BP was 127/80 to RA. Tongue was assessed by nurse due to client stated he thought he bit his tongue but no visual evidence on tongue. During fall client sustained an abrasion to area between nose and upper lip.
