RN CDNS contacted LPN on 9/30/2025 to notify her that immunization reconciliation was completed on 9/25/2025 and it was noted that Pediarix Lot # 9P329, expiration 3/6/26 showed 9 doses in computer, but the package was still sealed with 10 doses. Pediarix Lot # 39KS9, expired 9/7/2025 was still in refrigerator with two doses remaining. Computer expired inventory showed there should be 3 doses of expired vaccine. On 9/10/2025 LPN administered Pediarix to patient. On 9/30/25 RN Program Nurse was notified by RN CDNS of error. Per Program Nurse patient will need to be revaccinated ASAP, immunization record will need to be updated with the correct lot number and expiration date to show expired dose. An AS-8, VAERS and plan of correction will need to be completed. LPN notified Mom via phone at 2:28 p.m. of error and that dose will need to be repeated. Mom verbalized understanding and agreed to schedule child an appointment to be revaccinated.
