Symptom narrative

On 09/29/2025 a pediatric dose of Hep A was given to the patient by MA. The vaccine expired on 09/26/2025. Expiration date was discovered after the vaccine was given. Patient's parents are aware. Patient did not have any symptoms. Office manager and supervisor is aware. Plans in place: MA will document her lot numbers prior to giving the vaccine. MA will also have a second clinical Medical Assistant check the vials to ensure accuracy.
