Patient came to pharmacy 4 days later reporting a rash between her breasts. She stated the rash started 10 minutes after receiving vaccine, but was unable to come back as car would not start and had to be towed. She said her arm was swollen and felt hard. The arm did not look red or otherwise compromised with visual inspection. I recommended that she get checked out by a healthcare representative.
Pt reports no wbc's and so therefore has to get multiple flu shots every year (we denied giving her an additional flu shot). She is not currently seen by a physician for this and says she takes olive leaf herself to treat. She also reports black mold in lungs.

Patient circled no allergy on vaccine history form, but later said allergic to many things including Vit A and Niacin. Said she had a reaction to Shingles vaccine in past with blisters on both hands and tongue, but did not report it on vaccine history form (circled no)
