pt made an appointment for flu and shingles vaccine - it was typed in as flu and rsv and administered as such. When the patient asked when to return for the second shingles vaccine - the error was discovered. The pt was not upset and agreed that the RSV vaccine was ok to have but wanted to also get a shingles vaccine - The shingles vaccine was then input into the system an administered to the patient
