Symptom narrative

Shoulder Injury Related to Vaccine Administration. The patient developed persistent pain, paresthesia, and decreased mobility immediately following vaccine administration. She took Aleve without improvement. She was seen by Orthopedic Specialist who diagnosed her with SIRVA and administered a steroid injection which only provided temporary relief. I have offered her PT.
