Symptom narrative

Patient was mistakenly given a second Flu dose. She came in for a COVID-19 vaccination, along with her husband who wanted both Flu and COVID-19 vaccinations. Fluzone HD was processed for the patient and our pharmacy received a paid claim for medical insurance, which lead me (PharmD) that patient was eligible, without checking the WA state immunization registry. After the Fluzone HD was verified, the administering technician did not clarify with the patient she was receiving the Fluzone HD until after the shot was administered. As of 10/04/2025 at 2:26, I have followed up with the patient twice and she currently has no adverse reactions to report. She was encouraged however to contact the pharmacy if anything were to occur within the next few days and acknowledged the recommendation, thanking me for the call.
