Symptom narrative

Patient states he woke up at 1:00AM on 10-3-25 and "felt the urge to puke, but nothing came out" and he continued to "dry heave every 45 minutes to 1 hour." He "puked after every sip of water" and "didn't stop until about 8AM." By 9AM, patient said he started to gain an appetite.
