Symptom narrative

Concerns of allergic reaction. This occurred about 30 minutes prior to ED visit. Shortly afterward she complained of generalized pruritus, lightheadedness, and throat feels tight and itchy. No GI symptoms. No chest pain or shortness of breath, but has used her inhaler. No rash.

Medical history

Asthma, tachycardia, GERD & IBS, fibromyalgia, watching kidney function, HPV, pre-diabetic, Migraine and seizures, anxiety, depression, and PTSD

Other medications

Abilify 7.5mg PO Daily, Cymbalta 60 mg PO QHS, Lyrica 75mg PO TID, Topamax 100mg PO QHS, Trazodone 50mg PO QHS, ropinirole 1mg PO QHS, Seroquel 25mg PO daily, Acyclovir 800mg PO BID, Lumigan 1 drip Both eyes QHS, Symbicort 2 puff inhalation

Allergies

Wellbutrin- Convulsions, Povidine iodine (Betadine)-Anaphylactic shock, Soap (Betadine)- Anaphylactic shock, Venom honey bee- anaphylactic shock, Albuterol allergy-wheezing
