Symptom narrative

Px received 2 vaccines, both in left arm, 10/4/25. She reported to pharmacy upon opening on 10/6/25 to report that she experienced red rash that felt hot to the touch at the injection site. She stated that the rash started 10/4/25 and began getting bigger. It went from dime sized to half-dollar size by Monday morning. i asked her if she had any other symptoms. She said she did not. I advised her how to treat the site, monitor for worsening symptoms, and seek medical care if necessary. The px marked her arm to monitor the site. She stated that after another 2 hours, the site was enlarging, she now had a low-grade fever, and she felt dizzy upon standing. She was advised to seek medical help immediately. The px was taken to the local ER for treatment.
