Symptom narrative

Pt received vaccine on 9/24/25. Pt presented to pharmacy on 10/6/25 to report lingering arm pain, limited mobility in left arm after having first ever dose of 65 over Fluad. "can't play tennis, can't sleep on it." Pt made a point to confirm it was "that kid," our adult temp nurse helping provide vaccinations, as the person who administered the vaccine. Pt states the shot was given "way too high" and gestured to upper portion of deltoid muscle approaching shoulder blade. Pt was advised to let pharmacy know by other people/MD. Visualized pt can move arm in some capacity. Pt states she can't "lift it over her head." No formal diagnosis was provided other than pt was referred to see "orthopedic" and "had to see her chiropractor."
