Site: Pain at Injection Site-Mild, Systemic: Two injections given due to initial dose did not go into deltoid. See details.-Mild, Additional Details: During administration BD Integra Syringe with Retracting BD PrescrisionGlide Needle malfunctioned. Vaccine dose came out at needle connection to syringe. An additional dose was reconstituted and administered to patient in same arm due to initial dose not adminstered into deltoid of arm.
