Symptom narrative

Site: Pain at Injection Site-Severe, Site: Redness at Injection Site-Severe, Site: Swelling at Injection Site-Severe, Additional Details: Pt reported that her whole upper right arm was in pain, swollen, and completely hot. Pt also stated that she cannot use her arm, pain going down her elbow. She has been using cold packs and Ibuprofen 400 mg tabs 4 times daily without success. Advised pt to increase Ibuprofen dose w/ cold compress. If condition does not improve, see MD or go to Prompt Care/Hospital and report to MD.
