Site: Redness at Injection Site-Mild, Additional Details: pt only wanted flu shot initially. the nurse asked if his name and he answered yes. she went ahead and did both flu and covid shot for the pt but that were meant for the actual pt whose las name is different. pt doesnt want both shots at the same time due to not wanting severe side effect and will be on cruise soon. pt is later ok with getting both after we apologized of the miscommunication and they left. pt had no adverse reaction. had to choose one in order to submit the report
