Systemic: wrong vaccine dose given-Mild, Additional Details: pt came in for vaccines, but apparently had not gotten the right vaccine. she got the low dose flu shot not the high dose flu shot. she said someone called and apologized to her, but there is no way for me check documentation, so just covering my bases
