Site: Pain at Injection Site-Mild, Systemic: Chest Tightness / Heaviness / Pain-Mild, Systemic: Dizziness / Lightheadness-Severe, Systemic: Exhaustion / Lethargy-Mild, Systemic: Fainting / Unresponsive-Severe, Systemic: Flushed / Sweating-Severe, Systemic: Shakiness-Mild, Systemic: Tingling (specify: facial area, extemities)-Mild, Additional Details: After giving Covid-19 (Moderna) patient was fine. Also received slu shot (Fluarix) directly after that. He seemed a bit nervous but immediately stated that he is faint and feels like he was going to pass out. I called EMS after he fainted while sitting and verified that he was breathing fine still. He became rigid, stood up after 5 seconds and sat in chair until ambulance came to take him to emergency room for a check up.
