Symptom narrative

Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Nausea-Medium, Systemic: Shakiness-Mild, Additional Details: pt fainted after flu vaccine. EMS 911 called. Pt was unresponsive about 30 seconds but was aware of surroundings after episode. EMS asked her if she wanted to go the hospital. EMS took vitals had her stand and deemed her OK to leave as she was with a friend who was her driver.Pt declined and signed EMS paper. Pt left of her own volition.
