Site: Pain at Injection Site-Mild, Systemic: see details below-Mild, Systemic: Weakness-Mild, Additional Details: Nurse adminstered 1.5ml (3x dose) instead of 0.5ml. mistake was noticed right away and patient was told of error. Asked for his md information to include them on information regarding error, but new to area and no established md. spoke to pt about possible reactions and to go to ER/ urgent care if severe reaction.
