Symptom narrative

Systemic: pt moved away from needle, and needle-syringe came out of arm and vaccine liquid squirted all over arm-Mild, Additional Details: Pt was getting a flu shot and when the needle was inserted/plunger was initially being pushed, pt moved away from needle, which caused the needle/syringe to come out of pt's arm prematurely, causing one dose to be squirted on pt's arm, instead of injected. a 2nd dose (same lot/exp) was given, as pt received almost none of the liquid from the first dose, and the 2nd dose was injected successfully. A note was left on the first syringe, instructing pharmacy staff to damage it out.
