Systemic: none, just duplicate vaccine given-Mild, Additional Details: Patient was given a duplicate dose of covid vaccine on 10/3/25 at the (withheld name) nursing home. She received a dose from her doctor on 9/22/25 which was not documented in her chart, nor was billed to the insurance from the doctor on 9/22/25. The patient notified pharmacy after the shot was given because she did not remember before hand. It did not show up on the State registry. The primary physicin was called and notified of the duplicate vaccine.
