Symptom narrative

Site: Pain at Injection Site-Mild, Additional Details: Pt reports that he already had a previous HD Flu vaccine at his PCP office on 9/4, but was given another dose on 9/22 here in (withheld) during their COVID vaccine visit. Pt denies any serious adverse reaction but just came to clinic to report the doubling in the dose. Pt's wife stated that they did not know that they were getting another dose of the flu vaccine along with the requested COVID vaccine. She state that if they had known, they would not have agreed to get the flu shot again here in (withheld).
