Symptom narrative

Beginning Sunday 10/5/25 around noon, pt. noticed the reaction begin. It was on about half her upper arm. The reaction has continued to increase over the past few days. The entire upper arm is red, swollen, and feels warm to the touch. The pt. reports some pain at the site of injection. Both vaccines were given in her left arm side by side.

Current illness

Per pt. 10/7/25 9:35 am, she had not had any illnesses at time of vaccination and up to one month prior

Medical history

Per pt. 10/7/25 9:35 am, she has high blood pressure, arthritis, GERD

Other medications

Per pt. 10/7/25 9:35 am, she is taking celebrex, pregabalin, celexa, famotidine, losartan, prevagen OTC, aqua peace OTC (for sleep), cholestyramine. Pt. fills her meds at pharmacy.

Allergies

Per pt. 10/7/25 9:35 am, she is allergic to iodine dye and codeine
