Product administered to patient of inappropriate age

Symptom narrative

A patient came in with her husband, both to receive flu vaccines. Husband was over 65, and was due to be given the high dose fluzone vaccine. Patient was due to be given regular dosed fluzone due to not being 65 yet, she was 63. Both patient and her husband were called into the room together. During this time, the wrong fluzone was grabbed and administered to the patient. She received the high dose instead of the regular dose. Once I realized this, I went and grabbed another fluzone HD to administered to the husband. the regular dosed fluzone was placed back into the fridge. Pt was informed that she was given the higher dose vaccine. Pt seemed calm and was assured she would be okay.
