Symptom narrative

About 20 minutes after vaccine patient came back into clinic & stated she felt dizzy, lips started burning. She stated she took an antihistamine. Then about an hour later she came back & stated she felt itchy all over, but not unbearable & started to feel like her stomach started burning. We recommended for her to seek medical attention with an urgent care or ER. Or that we could call 911.
