Our pharmacy intern claimed that the patient did not get a full dose because of the device malfunction-either the pre-filled syringe or the needle. The syringe plunger was hard to press and did not reach the end completely. According to MERCK, dose repeat is recommended. The patient is aware of the situation and she is willing to get a repeat dose. No adverse effect was found, per patient.
